IAPEN India Association, IndiaTitle: The need for an interdisciplinary framework for the management of dysphagia
Abstract
Swallowing is a complex
neuromuscular process and difficulty in swallowing has been associated with
silent aspiration, pneumonia & mortality. Malnutrition is an independent
predictor of mortality in adults with dysphagia. Length of stay in hospitals,
rising medical costs, and caregiver burden are major concerns when it involves
patient care.
Working in
individual-specific roles does not result in timely positive outcomes when the
aim of the collective workforce involved in dysphagia care is to prevent
malnutrition, dehydration, and increased length of stay in hospitals and
nursing homes. The process of timely intervention and effective communication
eases the caregiver's burden and results in positive outcomes. Allied Health
Professionals should be made aware of not only their specific roles but also of
referral pathways and their scope of practice. It is also necessary to
understand other allied health roles so a definitive framework can be made for
the particular settings based on available resources and personnel. This helps
to find solutions with the available resources in limited community settings.
Interprofessional education, collaboration, teamwork, and developing a common
framework play an important role in the timely and effective interventions for
the management of dysphagia.
Interdisciplinary
collaboration and effective communication among nurses, Speech-language
pathologists, physiotherapists, occupational therapists, and dietitians, need
to be aware of their specific roles and roles in team collaboration, so the
best practice protocols can be established with effective tools and pathways of
communication are clear in each setting for effective management of dysphagia.
